Consultation
The initial consultation service is for the purpose of informing you about procedures,
costs, recovery time, and so on, and to review your medical condition and
medications. During this appointment, we will discuss with you a treatment
plan and thoroughly explain any surgical procedure that may be required.
Anesthesia options will also be discussed in detail.
Patients are referred for both medical and dental consultations.
You may choose to seek our treatment as a "self referral" or you
may be referred from a dentist or
physician. If your X-rays have been taken by your primary physician or
dentist, you may request that they forward them to our office. If time is limited,
please pick up the X-rays and bring them with
you. Otherwise, the required X-rays will be taken at the time of
your consultation. Often, surgery may be done the day of the consultation
visit. A parent or guardian must accompany all patients under the age of
18 at the consultation visit. The guardian must sign the operative consent at
the time of surgery.

We would appreciate the following information at the time of your
consultation:
- Your referral slip from your primary physician
or dentist and any X-Rays, if
applicable.
- A list of medications you are presently taking. If you are
taking Coumadin, we will need an INR drawn within 7 days of your
surgery.
- A list of medical conditions that may be of concern prior to
surgery (i.e. diabetes, high blood pressure, mitral valve prolapse,
heart murmur, artificial heart valve, etc.)
- If you have medical or dental insurance, bring the necessary
information in order to process your claims.

Payment Policy:
If you have dental or medical insurance that will cover your procedure, we ask that you pay what the insurance doesn't cover at that time
of service and we will assist with filing your insurance. If you do not have insurance, we expect 100% payment at the time of service. For
your convenience, we accept payments by Visa or MasterCard in our Lexington and Winchester offices.
